The fundamental work this plant does when seen from the perspective of a Nama Medicine Man is that it cleanses the toxins from our bodies. Either by inducing a vomit, or by means of a gentle, releasing laxative effect. Sceletium is not seen primarily as an inebriant by the Nama who use it in their pharmacopia, but as a plant that works it's healing way through the body.

Hence, a slight nausea might be felt, depending on your body's individual level of toxins. You may even throw up, but this will just be a bile-like liquid, it is just the Sceletium expelling the toxins from your body. If you do throw up you can bet the Sceletium has chosen this as the shortest route to expel the toxins from your system, you are therefore carrying a heavy toxic load and were actually in need of this cleanup. If you don't throw up you probably will feel the slight laxative effect Sceletium has as it cleanses the toxins from your body using the "lower road".

Also, these effects are dose specific.
Personally, I have only thrown up once, and this was with a large dose - 1 heaped tablespoon.
After throwing up and cleaning the body of toxins, a warm and pleasant feeling was experienced for the rest of that day.
It is unlikely that the usual dose will cause nausea.
